The Scarborough inter-professional primary care access model is a partnership to enhance care for the vulnerable, marginalized clients living in the Scarborough North and South Sub-regions. In this pilot, the priority population includes clients with symptoms of long term disability, who are unable to navigate the health care system independently and require access to inter-professional teams.

SCHC  is currently hiring for  2  full-time Registered Nurses  for their Community Health Team.

Key areas of responsibility:

The Registered Nurse works in collaboration with Ontario Works clients and their health care providers to collate information on a clients' disability. The information is carefully compiled and articulated in the Disability Determination Package (DDP) for adjudication to the Disability Adjudication Unit. Successful DDP applications make clients eligible for Ontario Disability Support Program (ODSP) benefits which offer the financial stability required to improve their long term health.

Educational and/or Professional Qualifications:

  • Undergraduate degree in nursing from an accredited University;
  • Registration and in good standing with the College of Nurses of Ontario;
  • Minimum three to five years' experience in direct client care;
  • Certificate/Diploma in Community Health Nursing considered an asset;
  • Current HCP CPR;
  • Superior advocacy, listening, oral and written skills;
  • Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and radicalized and LGBTQ communities;
  • Excellent interpersonal, communication, decision making, problem solving, organizational, conflict management and time management skills;

Sensitivity and awareness of culturally and racially, economically and socially diverse communities
